All of the issues lol. 

Video indicates bar path is not over the heel but closer to the toe, but coming up first and shoulders rounding forward, elbows not staying down, knees coming in. 

In general it's a mess. When I try low bar I feel the bar path over my heels and it is much better but I know that is only masking the issues of upper back tightness missing and other things. 

So I was just curious. Mucho appreciato

My guess is one important correction will fix most of the little junk too. Who knows what the hell that might be though... there are so many individual factors that go into how any given person squats. One thing that I go back to when things are feeling off or going to crap: I sit in a chair and stand up super slow paying attention to where all my parts are in relation to each other. Natural movements are efficient.
